This number, the ambulance rate per km, sounds simple, but it is only one part of the final fare. Quick answer: The ambulance rate per km is the price for each km after a fixed distance to begin with. The latest such instance is Pune’s revised fare order of August 2026, where the rate after the first 25 km varies from Rs 16 per km for a small van to Rs 30 per km for an AC ICU ambulance. Many people believe the fare is just distance times rate. Not really. A normal quote consists of three parts.
Base fare: A flat rate that includes the first few kilometres.
Per-km rate: the cost for each kilometre you drive after the base distance.
Extras: waiting time, night trips, tolls, oxygen refills, and similar.
The ambulance rate per km is applied after the base distance is used up. Say a provider charges Rs 1,200 for the first 25 km and Rs 18 for every km after that. A 40 km journey costs Rs 1,200 plus 15 km at Rs 18, so Rs 1,470. It is not Rs 18 for 40 km, and it is not Rs 1,200 plus 40 km at Rs 18.
Any quote can be checked against one formula:
Total fare = Base fare + ((Total km – Base km) * Per-km rate) + Extras
To do so:
Ask for the base fare and the km it covers.
After that distance, ask for the per-km rate.
Google Maps distance from pick-up point to hospital.
Determine if the trip is return or one-way. Do they charge return km?
Request a list of extras with amounts.
If the person on the phone cannot answer these five points clearly – take it as a warning sign.
The ambulance type is the most significant factor in determining the change in the ambulance charges per km. Operating a small van and a fully-equipped ICU vehicle are not the same price. According to the report of Punekar News and Pune Pulse in August 2026, the fare structure fixed by the Regional Transport Authority for Pune, Pimpri-Chinchwad and Baramati is as under.

In the Kerala notification of 2025, the minimum charge for the ICU and ventilator category will be Rs 2,500, Rs 50 per km thereafter, and Rs 350 per hour of waiting. The last state-level cap we found in Uttar Pradesh was an order from 2021 that set Rs 10 to Rs 30 per km after the first 10 km, depending on the type of ambulance. The ICU ambulance rate per km is higher than a basic vehicle’s, as the ambulance carries a ventilator, cardiac monitor, oxygen, and suction, and runs with a trained paramedic or doctor. In the Pune table, the AC ICU ambulance rates are Rs 30 per km, nearly double the Rs 16 of a small van.
Two things drive the cost up: the staff on board and the gear in use. If your patient only requires oxygen and a stretcher, a basic life support vehicle may be sufficient. Ask the doctor treating you which level is required, and read our emergency guide, ICU and basic ambulances before you decide.
Here’s how the ambulance rate per km compares on popular Lucknow routes. We use the sample numbers from the Bolero-type and ICU figures in the Pune table. These are not our tariff and not the Lucknow tariff.
The journey is still within the base distance. Bolero type: Rs. 1,200. I.C.U. Rs. 2,500. There is no charge per km.

Even when the per-km rate is clear, some items can add to the final bill. Ask about each of these before the trip:
Waiting charges: Many providers charge per hour after a free waiting period. This is important during admission or tests.
Late-hour trips: Night fares.
Long routes: Fees for tolls, parking, and state entry
refuelling, oxygen, use of a ventilator, or a doctor on board
Each item should be told up front, not added at the gate of the hospital.
The number of stops for the same route may vary with different providers. Common reasons include the age and model of the vehicle, the equipment and staff inside, the price of fuel, the time of day, and how fast the vehicle has to get to you. A very low quote is not necessarily a bargain. A driver who is not trained, or an oxygen supply that is missing, can cost you far more than whatever money you saved.
In a real emergency, don't waste time haggling. Get the car going. Get the written terms settled on the phone as it is on the way. Call first.

There is not a single national figure. Most providers charge a base fare, plus a per km charge. In the 2026 order for Pune, the rate ranges from Rs 16 to Rs 30 per km depending on the type of vehicle. In the ICU category in Kerala, it is Rs 50 per km. Rates can be different in your city.

Normally, the route distance from pickup to drop-off is used. Return km may or may not be charged, so discuss before the trip.
